First Visit

Your first visit to our office will be in the form of a consultation. The physician will meet with you to discuss your gastrointestinal issues, medical and social history, and to perform a physical examination. They will then discuss with you a treatment plan and course of action.

Please Bring The Following Items With You To Your Visit

  • Most current insurance card(s)
  • Driver’s license or photo identification
  • Co-payment in the form of cash, check or credit card
  • A complete listing of your medication, including dosages
  • If you have seen a Gastroenterologist in the past and/or have had prior diagnostic testing done CT scan, x-ray, lab work, etc.), please have those records forwarded to us in advance or bring with you to your appointment.
  • Ensure that our office has received a referral, if necessary, in advance of your appointment.

Insurance & Billing

Please contact our office to verify the acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure and insurance plan. We are happy to file insurance for your reimbursement as long as you are free to choose your own gastroenterologist. For out-of-network services, the estimated amount a patient will be billed is available upon request.

Referrals are patient responsibility. If you do not have your referral or if your primary physician did not enter one into the system, you will be rescheduled for your appointment.

Our office is open weekdays from 9 to 5PM Eastern.

Please call our office and request an RX refill through our staff.

For new patients, please download, print, complete the following forms and bring them to your first appointment to save yourself time! Please remember to bring in your insurance card, Photo ID and a referral if it is required by your insurance.

For serious emergencies, please dial 911. For other urgent care matters, seek out an emergency facility. For after hours care our answering service will direct your call to the ON-call physician.

If you have a medical emergency, please seek urgent medical care with 911, or find the nearest emergency facility.

We have partnered with Tandem for prior authorization prescription checks!

As part of our commitment to improving patient outcomes and efficiency, we are now using a seamless and efficient solution to handle prior authorization checks before sending prescriptions to a local pharmacy. 
 
How it works:
1. Your doctor sends your prescription to Tandem where you can track the status anytime.
2. Tandem works to get you insurance coverage and check coupons and prices.
3. Tandem sends the prescription to your pharmacy of choice for pick up or delivery.
